Here are the instructions for requesting credentials by scanning the provided QR code:

1. Open the QR code scanner app on your mobile device.

2. Position your device's camera over the QR code provided.

3. Ensure that the QR code is fully within the frame and wait for the scanner to detect it.

4. Once the QR code is successfully scanned, you will be redirected to the credential request form.

5. Fill out the required information accurately and completely.

6. Review the information entered to ensure its accuracy.

7. Submit the request by tapping the submit button on the page.

8. Wait for email confirmation that your credential request has been received and processed.

9. You may  also click the "Request Here" button to access the google request form.
